When a child is accompanied by an adult other than a parent
or legal guardian, a temporary card will be issued and an application
and a letter of explanation will be sent to the parent or legal

along with the original mailing envelope will serve as parental
permission for the temporary status to be removed.
When the custody of a child is shared by parents with separate

two cards may be issued, one for each address/custodian.
